Whether you need a DOT medical card for trucking, transit, delivery, or any commercial driving role, our team is here to guide you every step of the way.
Dr. Himanshu Pandya, MD, is a licensed medical examiner certified by the FMCSA (Federal Motor Carrier Safety Administration) to perform DOT physical exams and issue valid DOT medical cards. If you hold a commercial driver’s license (CDL) or operate a commercial vehicle, you are required to undergo regular DOT physicals — and we make the process simple, efficient, and thorough.
Floral Park Medical and Cross Island Medical are one-stop destinations for all DOT physical examination assessments, ensuring the following in their service delivery.
Call us at (718) 831-6600 (Floral Park, NY) or (718) 424-2457 (Jackson Heights, NY).
When you schedule your appointment, the team at Floral Park Medical, P.C. and Cross Island Medical will let you know what to bring for your DOT physical exam. To help make your visit smooth and complete, please remember to bring the following:
If you have diabetes, please bring a copy of your most recent hemoglobin A1C test. If you have a heart condition, we request a note from your cardiologist outlining your heart health history. These documents help our team provide a thorough and accurate evaluation.
Not all drivers who need a DOT physical have a commercial driver’s license. Different types of CDLs allow drivers to operate specific types of commercial vehicles. You should ask our doctors if you need a CDL physical near Floral Park and Jackson Heights or a DOT physical is sufficient as per your requirements.
If you have any questions regarding what a DOT physical examination entails, our staff will walk you through every step of the process.

Not by default. A standard DOT physical does not include a drug test, but your employer may require it separately as part of pre-employment or random testing protocols.
If you don’t meet the medical standards, you’ll be given reasons and may be eligible to reapply after addressing the issues, or request a medical exemption if applicable.
Floral Park Medical offers DOT physicals at two convenient locations: Floral Park (257‑20 Hillside Avenue) and Jackson Heights (34‑57 82nd Street, Suite 1G).
The exam includes a thorough medical history review, vital signs (e.g., blood pressure), a full physical assessment of major systems, and often a urinalysis to check for diabetes or kidney-related conditions.
Any driver holding a commercial driver’s license (CDL) or operating commercial vehicles—such as those carrying 15+ passengers, hazardous materials, or weighing over 10,000 pounds—must undergo a DOT physical.
When you come in for your DOT physical, bring your prescription eyeglasses or contact lenses, hearing aids if you use them, a list of medications with dosages, and the names and addresses of your healthcare providers. If you have diabetes, bring your latest A1C test; if you have a heart condition, bring a note from your cardiologist.
Dr. Himanshu Pandya will review your medical history, check your vital signs, and perform a full physical exam. A urinalysis may be done to screen for kidney problems or diabetes. If you meet all medical requirements, you’ll receive a DOT medical card, typically valid for up to 24 months.
